Mark - I have an old version of the Orvis sling pack. Not the guide one, the older and smaller one (Blue and grey). I like it, but I wish it was larger so it could hold a water bottle in addition to my fishing gear.
I'm a right-handed caster and I wear the pack on my right shoulder without issue. One thing I'll say is that the cross-body strap (separate from the shoulder strap) really helps to keep it firmly I place.
I think my favorite part of it is having my forceps in a holster on my chest - built in to the shoulder strap.
