I've fished the NBPR on a number of occasions over the last few years. I've found that Barnum, WV is a nice place to go. Like Terry, I've had really good days and bad days. The dry fly fishing is usually hit or miss. You pretty much have to be there during hatch to find fish on dries. Nymphing however can work year round. Prince nymphs, zebra midges, and tan scuds are my go to there as well as on the Savage. I live in DC and usually make it an overnight trip and either camp up near the Savage or at Barnum. From Barnum you can hike up river to the dam and fish the two miles of catch and release water (really scenic) or hike down river. There is a road that parallels the river for miles down from Barnum so its fairly easy access.
With that being said, there are really nice trout all the way down to the paper plant at Bloomington, MD. If you can take the smell, you can fish there or even farther down river. There are nice trout and smallmouth below the paper mill.
