I think you are going to have to do the mix and match yourself.  I would look for fishing first and then do a google for sporting clays in the area.  It's going to be easier to find a sporting clays range than good trout fishing.

I am guessing you will have to drive to the range.  I can think of a few places that I would start.

* For SNP/brookie fishing: Rose River Farm was mentioned, there or Graves Mountain Lodge would be good place to start.

* Harman's cabins in WV would be good a little further away.

* Carlisle PA (Yellow Breeches, LeTort and Big Spring) The Allenberry resort is under new management and might be an interesting place.  I haven't heard any recent reviews.

* Central PA,  Spruce, Little Juniatta, SpringCreek, ....

It sounds like a fun outing.
